If you have obsessive-compulsive disorder (OCD), you know how overwhelming intrusive thoughts can be. OCD is made up of obsessions (unwanted thoughts, images, or urges that create anxiety) and compulsions (behaviors or mental rituals people do to try to feel certain or safe). While compulsions may provide temporary relief, they actually reinforce OCD, making the cycle stronger over time.
Exposure and Response Prevention (ERP) is the gold-standard treatment for OCD. In ERP, you gradually face situations, thoughts, or sensations that trigger your anxiety (exposure) while resisting the urge to engage in compulsions (response prevention). Rather than trying to get rid of intrusive thoughts, ERP helps you learn that you can tolerate uncertainty and anxiety without letting OCD dictate your actions.
For example, someone with Health OCD might notice a mild headache and immediately worry that it means they have a serious illness. Their OCD may urge them to Google symptoms, check their body repeatedly, or seek reassurance from loved ones. In ERP, they might practice delaying the urge to Google, resisting reassurance seeking, or allowing the thought, “maybe it’s something serious, maybe it isn’t,” without trying to find certainty. Over time, the brain learns that the anxiety naturally fades even without performing compulsions.
ERP isn’t about ignoring real health concerns or proving that your fears are impossible. Instead, it’s about changing your relationship with uncertainty so OCD no longer controls your life. With practice and support, many people find they spend less time trapped in the OCD cycle and more time focused on what truly matters to them.
